| Description |
The goal of the immunization promotion program is to reduce disparities in immunization coverage among minority children living in New York City, NY. A coalition of 23 community organizations designed and implemented the program.
The immunization promotion program was integrated into existing community-based social service programs. In order to ensure sustainability, immunization information was incorporated into six areas: child care, parenting guidance and assistance, facilitated enrollment for health insurance, clinic referrals, housing assistance, and the Supplemental Feeding Program for WIC. An implementation guide was developed to help peer educators at the various community organizations modify their activities to include immunization promotion.

| Results / Accomplishments |
Between 2000 and 2007, the coalition provided outreach to 31,156 parents and enrolled 10,251 children in the program. As of 2007, the average immunization coverage rate for the coalition was 98.6%, an increase from 63% in 2002. |
