About Us

The Healthy Montgomery community health improvement process is a community-based effort to improve the health and well-being of Montgomery County residents. Healthy Montgomery is under the leadership of the Healthy Montgomery Steering Committee, which includes planners, policy makers, health and social service providers and community members. The community health improvement process includes data collection, identification of areas for improvement, priority-setting, strategic planning, implementation planning, and collaborative efforts to address the priority needs in Montgomery County and evaluate the success of the improvement efforts.

Healthy Montgomery Steering Committee Meetings
 
The Healthy Montgomery Steering Committee (HMSC) meets quarterly to direct the countywide effort to complete all phases of the community health improvement process. Our inaugural cycle begain in June 2009 and is anticipated to be completed in December 2012. when we moveinto monitoring and evaluation of our areas of focus.

Action Planning 

Our Action Planning phase includes planning, implementation, and evaluation activities.

During Planning, Healthy Montgomery convenes partners and stakeholders to formalize planning process (evaluate membership); maps assets, resources, and best practices; convene content experts to form workgroups; identify strategies; set goals for strategies; and identify objectives to achieve goals.

During the Implementation, Healthy Montgomery work groups evaluate opportunities for intervention; address barriers; recruit additional partners; implement action plan; monitor plan; and communicate progress with stakeholders and public.

And for Evaluation, Healthy Montgomery work groups create an evaluation plan for actions adopted; gather data/information to document issue areas, activities implemented, and progress on achieving objectives; provide conclusions with justifications; share lessons learned; and inform changes to future action plans.
